GRADING SCALE DOCUMENT! HELP!

Hi everybody.

I'm with a problem and maybe somebody can help me.
USA has asked to send my GRADING SCALE.
As I graduate in a foreign University, there is no grading scale document.
What I can do is ask my course coordinator to make a document with the necessary information.

WHAT I NEED IS:
AN EXAMPLE of a GRADING SCALE DOCUMENT where he can use as a reference to do this document.

There is anybody that can help me? I have 2 WEEKS to send this document!

Generally North American schools use a 4.0 grading scale and letter grades convert to points (A=4.0, the highest).
You might want to call the schools you're applying to to get specifics on your country's scale-U.S scale.

I saw these requirements. But, what I actually need is a STANDARD of this document. I mean, the proper document as an image or word doc, to send my course coordinator, so he can use as a reference and write one.
The other documents I already sent. Is it possible that they are asking ONLY for a documente that says:
STUDENT X has the average 7.5?

I think they just need to know how your average converts to their school's system, so if a 7.5 equals a 3.6 for ex, that's what they need, and to see your official transcripts of course. Those would be mailed hard copy, no screen shots, computer images or anything like that.

But I really don't know if a school needs a kind of special document other than that, or if they have any to use as examples. I think your best bet is to call them, good luck!
